Where's your M-spike hover?

For those of you that don't have a zero m-spike, but your disease remains stable, where does your m-spike hover?

I started at0 .9 in April 2012. Six months of Velcade and Dex (mine dropped to 0.4 after two months) and now nine months on just Rev mine has hovered at 0.4to 0.2. PResently taking 5mg of REv daily.

Coach,

Very good numbers! My husband Frank, age 70, was diagnosed in January of this year. After 6 months of Dex & Velcade, his m-spike is less than 5%. Doctor is calling this a "very good partial remission." He started out with 80% malignant plasma cells. Since he is unsure about proceeding with a SCT, he is continuing on with the same induction therapy for another 2 months, and then we will see where we go from there.

Questions: Did you have a transplant?
How are you tolerating the Revlimid? Any major/minor side effects?

Tolerating Revlimid pretty well (some insomnia and horrible taste in mouth, some fatigue) I'm 72 and did not have transplant and don' t plan to.

Mine re-appeared in June of 2012 after being free for a little more than 2 years. It started at 0.1 and has increased to .35 over the past year. I had been on a maintenance dose of Revlimid, but have been off since January of this year. Since all other indicators are normal, we're just watching it quarterly.

Thanks Coach for the helpful information. It's nice to hear from others who are responding well with maintenance therapy, minus the stem cell. So much of what I read includes a stem cell at some point, but my husband is in agreement with you, and does not plan to.

It is refreshing to see something on the Beacon re: people with multiple myeloma who did not do the stem cell transplant..
I know many mature people don't use the computer/internet, therefore they are not joining your discussions.

PLEASE tell me if the lab labeled ---BETA -2 - Microglobulins --Is where you get the results for
M- spikes? Also my husbands Beta 2 Microglobulins, serum is 8.19...
His IgA is 3270 and that scares me. Plus his other labs are all scary.
Please consider being more detailed re: what is going on with your type of multiple myeloma from what I understand there is Kappa and Lambda catagories of multiple myeloma. It would very helpful if that is indicated in their information.

In 2010 it started out at 0.5 and has crept up to 1.1 this year. Being treated with Zometa and bi-monthly blood work blood work and annual, coming up in September with Dr. B in Little Rock.

My diagnosis is multiple myeloma, IgG-Kappa type
Initial biopsy showed 30% plasma cells.
Free Kappa 423 ( normal 3.3-19.4)
M-spike 0.9 (normal is 0)
Nothing else in blood was abnormal (including cbc, glucose, albumin, etc. and other immunoglobulin values)
Many small lytic lesions on spine (amazingly, no pain)
Induction with Velcade and Dex (I hated the dex) resulted in partial remission 0.4 M-spike and 19 Free Kappa
Now maintainance with Revlimid 15mg, then 10, now 5mg. Last M-spike was 0.2.
No plans for transplants or any other therapy at this point.

The m-spike is found in the results from the SPEP test. This is a copy of the items that are documented in the lab report that I get. The SPEP interpretation at the end is the m-spike. Other labs may document the results differently. You may not be getting the full results if you are only getting the results from the fractions and not the interpretation.
SPEP TP
Albumin Fraction
Alpha1-Globulin Frac
Alpha2-Globulin Frac
Beta-Globulin Fracti
Gamma-Globulin Fract
SPEP INTERPRETATION Serum protein electrophoresis shows a band in the gamma (or other) region that is quantitated to be ___ g/dL.This band is consistent with the previously identified ___ (IgG, IgM, IgD) lambda, or kappa. There is, or isn't,immune paresis.

The analysis of a fresh 24 hour urine collection for the detection of
Bence-Jones protein is recommended, if it has not already been requested.
