Question about Kappa light chain ratio

Hello,

I have multiple myeloma and was diagnosed with Iga Kappa in April 2010. Ive had two Auto SCTs in Sept 2010 and March 2011 and have been in a complete remission for 2 years. Last week, my dr advised me that my bloodwork looks perfect but my kappa light chain ratio has slightly risen to above normal levels, even though no m spike is detected and my iga is normal. Any idea would is going on?

See the discussion on a related topic. I think it will help answer your questions. It sounds like your Dr. is on top of it. A minor increase in the IGG Kappa may not mean anything. An upward trend is what you don't want to see, but even then, it may be driven back to normal levels with an adjustment to your current protocal.

If it helps any, Im currently not on anynsort of treatment, other than Zometa. Im 34 and am relatively healthy if that means anything.

Also, my ratio is around 22 where the norm is below 19 or so. If i have no m spike and my iga is fine, am i technically relapsing?

I think its too early to jump to any conculusions about relapse. Your Dr. didn't use that term so I certainly wouldn't.

But if its not a relapse, any other ideas what may be going on?

This will need to be monitored somewhat closely. Relapse can occur without the intact immunoglobulin that was seen previously (IgA).

However, you can have fluctuations in the free light chians that are not indicative of relapse and sometime are associated with your immune system regrowing after transplants.

I would recheck light chians monthly for now.

Thanks for the replies. Sounds like this is just something that will have to be watched, even though my m spike is undetectable and my IGA's are all within the normal range.

Any other takers on what may be occuring in my body? I hate knowing that I'm in a Stringent CR but now because of the Free Kappa Light Chains being slightly above normal along with the ratio, I'm freaking out. :?

Hello,
I have been with the Myeloma disease for 7 years. My current treatment of Velcade and Dex are going on 30 months. My last FLC Ratio was 118.30 ( I had a bad cold)and had trended higher over the last three tests. I have had a 115.00 number earlier in the year and my average from Jan to Jan is about 75. The IGG is very slowly climbing but is still onlt 2025 and the m-spike is 1.2 and also slowly climbing?
Two questions:
Is the 75-115 too high and can you manage the disease without problems at this level? I am going to discuss with Onc in two weeks but it seems maybe we should be thinking about adding another drug to the mix and bring the numbers down to at at least a Partial response?
Ny onc balances the QOL with the treatment and so far we have been good?

VAD and SCT in May 2006
CR for 4 years and since on Vel /Dex weekly( 1Mg and 10 Mg Dex)
No Bone or Kidney issues.
