Newly diagnosed with MGUS, worried it may be progressing

by Gemsy2481 on Mon Jul 04, 2016 10:48 pm

Hi,

I'm a newly diagnosed 35 year old female who has been diagnosed with MGUS in February. However, my recent blood results show anemia and an increase in my IgG kappa light chains and ratio. My consultant doesn't suggest a bone marrow biopsy yet. However, due to some back pain, he has ordered an MRI scan this week. My bloods are as follows:

WBC: 4.6
RBC: 3.53
Hemoglobin: 115
HCT: 0.38
MCV: 109
MCH: 32.6

M-spike: 6.9

Immunoglobulin IgG: 16.1
Immunoglobulin IgA: 2.07
Immunoglobulin IgM: 1.10

Serum free kappa light chain; 117
Serum free lambda light chain: 16.4
Serum free kappa/lambda ratio: 7.13

Would these results suggest progression to smoldering multiple myeloma? Is anyone else in a similar situation or could give some advice?

Thank you

Gemsy2481,

Welcome to the forum.

One can't say if one is progressing based on only one sample of data. How do these numbers compare to what was measured back in February (and in between)?

Also, unless some marker(s) really takes off in a big way in the wrong direction, one generally looks at the results of a few tests before making a call that one may be progressing. That is because a lot of these markers will naturally fluctuate considerably from day to day and month to month. These graphs might give you an idea of just how much some of my markers fluctuate from test to test:
