I was diagnosed with smoldering myeloma 8 years ago. I have regular doctor visits and have been somewhat stable during that time. I went to the doctor on Tuesday and my test results are coming in a few at a time. I do not have the M-spike information, but my kappa / lambda ratio seems concerning.
Oct. 2013: 1.88
Oct. 2014: 2.39
Oct. 2015: 2.71
Feb. 2016: 3.34
May 2016: 6.34
I am wondering if my latest kappa / lambda ratio is significant.

Re: High kappa-lambda ratio in smoldering multiple myeloma
That's still a very low ratio as FLC ratios go for somebody with a plasma cell disorder. What marker put you into the smoldering multiple myeloma category as opposed to the MGUS category?
As a matter of course, my FLC ratio bounces all over the place. The link below will give you an idea of why I take test-to-test bumps in my markers in stride these days. I will only start to get concerned if there is a sustained and very significant negative trend in one of my key markers.

Re: High kappa-lambda ratio in smoldering multiple myeloma
I believe the IgG kappa gave me this diagnosis, but I am not sure. I have not kept up like I should. Blissful ignorance. Originally I was given an MGUS diagnosis, but several doctors later (retirements and moves for previous docs), my diagnosis was changed to smoldering myeloma.
Thank you for the quick response. I have just been seeing a steady increase for a year and a half and this one caught me off guard.

Re: High kappa-lambda ratio in smoldering multiple myeloma
I believe that when your light chain ratio exceeds 8, along with other risk factors, you'd be considered high risk smoldering, for which you would be eligible for clincal trials
