deletion of chromosome 13

When I was diagnosed with multiple myeloma after a spinal fracture, a fish test was performed it showed that I had a deletion 13. after 6 months of Velcade, dex and doxsil my oncoloigist told me my deletion 13 does not show up any longer in my labs, and that I no longer have a deletion 13. I'm confused, how does that happen ? thank you vince

Deletion of chromosome 13 is much more common in the FISH test than it is by other methods. I read someplace that 40 percent of multiple myeloma patients will show a chromosome 13 deletion on a FISH test. However, one is usually considered high risk if del 13 shows up on cytogenetic tests other than FISH.

chromosome 13 is involved in inhibiting cancerous cell growth. In multiple myeloma 13q deletion is frequently seen on the cancerous plasma cells . In my case 7% of my myeloma cells had 13q deletion, I had 48% myeloma cells in my marrow. Velcade is especially effective in patients with 13q deletion,is thought to mitigate its effect as an adverse prognostic marker.
Likely means that the normal plasma cells in your bone marrow do not show this, though your tumour cells did.

I was diagnosed with tcell 13 deletion and also have type A, which the transplant oncologist at the Univ. of Iowa told me put me in a higher risk group. He said I would relapse quickly after stem cell transplantation and would likely die within 3 years. At this point we decided to go to the Multiple Myeloma Institute for Research and Therapy at the Univ. of Arkansas for Medical Sciences for a second opinion. The oncologist there told me that having or not having tcell 13 deletion did not seem to impact treatment outcomes on their protocols. As I had had prior treatment which can improve your gene array results (mine were testing low risk at the time), I was placed on a more rigorous protocol. It is my understanding that the gene array can change at any time and you can go from low risk to high risk.

We decided to go with MIRT at UAMS, though it was very difficult to be away from home for five months for treatment. The odds of basic five year survival are doubled at UAMS compared to nationally.

The doctor who has been spearheading the research and treatment there, Dr. Bart Barlogie, received a national Physician of the Year award for his work with multiple myeloma.

Deletion of chromosome 13 by FISH is not an adverse prognostic feature, as more myeloma patients have this than those who don't.. It is debatable whether deletion of 13 by conventional cytogenetics of the karyotype is adverse. Bortezomib has been shown to overcome this.

Is deletion of chromosome 13 by FISH but a normal karyotype by conventional cytogenetics considered high risk?

Hi Perseverance,

Great question: The two tests: classical cytogenetics and Fluorescence In Situ Hybridization (FISH) can detect chromosome 13 abnormalities. FISH picks up more 13 abnormalities than classical cytogenetics so it is a more sensitive test. Around 5 to 15% of multiple myeloma patients will present with chromosome 13 abnormalities by classical cytogenetics and a higher percentage of patients can have this abnormality seen by FISH. The 13 chromosome deletion (del 13) had been associated with a lower response rate and decreased overall survival in the past. It is thought that del 13 as detected by classical cytogenetics confers a worse prognosis that those detected by FISH. However, the use of bortezomib as part of upfront therapy has decreased del 13 as a serious risk factor. Some clinical investigators think this is still as risk factor for overall survival even with the use of bortezomib. Also del 13 can be seen with 4:14 translocation or del 17, both of which are associated with worse overall survival. The use of new agents as well as the combinations of new agents along with stem cell transplantation may change prognosis. These cytogenetic tests and perhaps molecular testing may help clinicians make recommendations regarding the aggressiveness of approach and type of treatment.

Before I had my bone marow biopsy, I was told that cytology testing, etc., would be done. However, my bone marrow showed 26 percent plasma cells, but no cytology or FISH testing was done. Has anyone else experienced this? I have HMO insurance plan.

My nephew Jon was diagnosed with multiple myeloma in july 2011 and fought it with all the courage and strength he passed away in February 2012 8 months later. He was an enviromental engineer. My question is can exposure to cancer causing toxins cause a chromosome 13 deletion.

I am sorry to hear about your loss. How old was he?

No correlation between environment and multiple myeloma has really been found concretely.

Some think heavy exposure to the petroleum industry may have a link but noting is concrete with that either.
