Change in kappa-lambda ratio - is my MGUS progressing?

by jjnyc on Mon Apr 03, 2017 12:22 pm

I was first diagnosed with MGUS in 2007. My M-spike remained at 0.3 g/dL (3 g/L) until last September when it went to 0.6 g/dL. My kappa-lambda free light chains and ratio have remained in the normal range. However this result showed that the ratio is now 0.76. In September it was 0.56. I am still waiting to hear if the M-spike itself has increased at this check-up.

My question is, in general, do these results seem to indicate progression?

I am meeting with my hematologist on Thursday, but I am trying to get a handle on these changes and whether or not they are necessarily significant.

Thank you!

Hi jjnyc:

I'm not a doctor, but I wouldn't take this as sign of progression unless you really start to develop a pretty serious trend over a few lab tests.

The disease markers in MGUS and smoldering multiple myeloma patients tend to naturally fluctuate quite a bit, and I've gotten used to this over the years. As an example from a smoldering multiple myeloma patient, see my chart history here:
