Worried about multiple myeloma, confused by lab results

by zumanga1970 on Tue Sep 29, 2015 8:34 pm

Before any test was done, my hematologist diagnosed me with MGUS and I have an elevated protein in my blood. I have mild anemia. She said that my RBC and WBC are not that low and platelets are normal. I have scleroderma, Raynaud's, Marfan, hypothyroid, and GERD.

Can these results also be cause of these conditions too?

Elevated SED rate 100
Kappa free light chain, urine 307.00
Lambda free light chain, urine 28.70
Kappa / lambda free light chain ratio, urine 10.70

Kappa free light chain, serum 202.6
Lambda free light chain, serum 81.9
Kappa/lambda free light chain ratio 2.47

The other tests I can understand. I have the results from an online program. I have an appointment in a week. Just want to understand and want an opinion. I will read more about it. Any input would be good.

Hi Zumanga,

Welcome to the forum. You certainly have a lot going on.

What are the units of measure for your serum kappa and lambda numbers, and what are the normal reference ranges for those figures? These should all be on the test results you are looking at.

Sorry about that. In the normal range it states MG/L for urine and serum. Thank you for being the first to respond to my post.

Yes, but for the values below:

Kappa free light chain, serum 202.6
Lambda free light chain, serum 81.9
Kappa/lambda free light chain ratio 2.47

There should be set of numbers next to the above numbers that indicate what the normal ranges are for each of these values. What are they?

Also, what is your serum creatinine level?

Sorry again, I am just trying to keep my head up. these are it

kappa light chains free urine normal range is {1.35-24.19} mg/L
lambda light chains free urine normal range is { 0.24-6.66} mg/L
kappa/lambda free ratio urine normal range is {2.04-10.37}

free kappa serum normal is {3.3-19.4} mg/L
free lambda serum normal is {5.7-26.3}mg/L
kappa/lambda ratio normal is {0.26-1.65}mg/L

My doctor said my creatinine levels are normal and the primary diagnosis is monoclonal gammopathy. I have to go back to her in 2 weeks cause i just got the results a few days ago and she didn't call me about it because i have an app soon.

Thank you again

I'm not a doc, so I really can't speak to what the other diseases may be doing to your numbers.

Your serum kappa, lambda and kappa/lambda ratio are all high. Putting aside the other diseases and knowing that you already have a diagnosis of MGUS, all these marker levels "might" suggest a monoclonal gammopathy with some renal impairment. That's why I was asking about your serum creatinine level (which is kidney function marker).

You obviously have a complex case with all these various diseases and your hematologist is best suited to comment on these results on the basis of having access to your overall chart and history.

Wish I could be more help, but this isn't really something that one can comment on very well on the Internet.

Thank u so much. Yes I figured that. It could be also going on with one of my other inflammatory disease too. I will wait until my doctors app and I will post and update. Thank You so much for your help. I appreciate it so much.
