Wondering if lab results mean I have multiple myeloma

Hi peoples!

Like a lot of you, I never thought I would be visiting a place like this, But, here I am. Hoping someone can explain these results, cause I am so lost!

Stared off with incidental finding on MRI of a bone marrow replacing process in left arm. Went for CT scan, no bone destructive lesions seen, but am told I have a lesion in arm.

Had a protein, total and protein electrophoresis, 24 hour urine and immunofixation test done from Quest Diagnostics. Results:

Albumin 37%
Alpha-1-Globulins 3%
Alpha-2-Globulins 8%
Beta Globulins 12%
Gamma Globulins 41%
Says normal pattern

Immunofixation, Urine No Monoclonal protein Detected

Creatinine, 24 hour urine 2.43
Protein/Creatinine ratio Y
Protein, Total, 24 hour 100

Protein Electrophoresis and Kappa/Lambda light chains, serum:

Protein total 7.7 (6.1-8.1 g/dL)
Albumin 4.4 (3.8-4.8 g/dL)
Alpha 1 Globulin 0.4 (0.2-0.3 g/dL)
Alpha 2 Globulin 0.6 (0.5-0.9 g/dL)
Beta 1 Globulin 0.6 (0.4-0.6 g/dL)
Beta 2 Globulin 0.5 (0.2-0.5 g/dL)
Gamma Globulin 1.2 (0.8-1.7 g/dL)

Kappa 349 H (74-295 mg/dl)
Lambda 168 H (32-156 mg/dl)
Kappa/Lambda ratio 2.1 (1.3-2.7)


Met with myeloma doctor who after looking at all results felt that this might be a marrow conversion. I am very clumsy and did fall and land on the area where the lesion is said to be.

The more I look at this, the more I get confused. Any input would be great. Thanks and good luck to all!

Hi there,

I'm a relative newbie to the myeloma world, so please take everything I say with a grain of salt, but from what I've learned thus far:

Your urine electrophoresis did not seem to detect an M-Spike (good news)
Your urine immunofixation did not detect any monoclonal protein (good news)

I don't really know enough about the urinary creatinine-to-protein ratio to comment, sorry.

I don't see whether or not an M-spike was detected on your serum electrophoresis, but I'm assuming not, since your doc would definitely be doing further workup on you. Take a look at your lab report and see if it mentions anything about an M-spike, M-protein, or paraprotein, as these are all more or less synonyms for the same thing.

Your serum free light chains are both elevated, but the ratio is normal, which implies that their elevation is most likely polyclonal in nature (i.e., probably due to something besides myeloma, MGUS, amyloidosis, or another disorder that is monoclonal in nature), which is good news.

Did your doc happen to run either of the following: serum immunofixation or quantitative immunoglobulins?

Hey Bdtyq5,
Thanks so much for your reply. The only other thing I can add is my sed rate was 1,and everything on my Comprehensive Metabolic Panel and CBC were all normal, don`t know if that helps. Did get a bone marrow biopsy a couple of days ago. Takes up to 10 days for results, so just sitting here thinking about this whole thing. Hope your situation is going well for you. Once again, thanks for your input.
