What is oligoclonal gammopathy?

Hello all,

My mother already has osteoporosis, a fatty liver, Vitamin D deficiency and high blood pressure. She's 61 and over weighted. She's under control having regular blood tests etc and using medicine for these, under the controls of endocrinology and gastroenterology.

Here is her recent test results and I am afraid that she might have multiple myeloma. Please help me by looking at these results. I don't understand. Thanks.

It says that there is suspicious view on the gamma zone and suspicious view of lgG kappa and lgG lambda oligocolonal gammopathy. I have attached the images. Thanks in advance.

Hi Redblue,

Sorry to hear about your mother's situation.

You can't definitively say that your mom has any stage of multiple myeloma from these test results alone, but the results do strongly suggest that she should have some further tests , preferably under the guidance of a multiple myeloma specialist.

With various stages of multiple myeloma, patients usually present with one elevated immunglobulin (IgG, IgA, etc) in conjunction with elevated free light chain (lambda or kappa). When these elevated levels are due to the same cells making exact, nonfunctional copies of themselves, that is known as a monoclonal gammopathy. When two or more pairs of an immunoglobulin and a free light chain are making nonfunctional copies of themselves and are elevated (in this case IgG with Kappa and IgG with Lambda), that is known as oligoclonal gammopathy. Oligoclonal gammopathy is not any worse than having a monoclonal gammopathy, so don't fret over this particular variation of the test results. All the literature that pertains to monoclonal gammopathies would still apply in this case.

But again, there are some additional tests that need to be done, including an immunofixation (IFE) test, a bone marrow biopsy and a PET/CT and/or MRI to verify if this is truly some stage of multiple myeloma. Note that even if this is a monoclonal or oligoclonal gammopathy , this may only be an early stage such as MGUS that may never require treatment and your mom may never be any the worse for it.

If you let us know what city your mom lives in, we can make some recommendations on where to find some top multiple myeloma specialists.

I am so glad to see a response from you. I have been concerned that your wise analysis has been missing for quite some time. I hope you are well and have been away for a pleasurable reason and not from health issues.
