What does CKD Stage 3 mean?

Hi.

Keith finished 6 cycles of treatment (the last two only because our transplant team wasn't ready). In the last visit with his doctor before going to the team, she noticed a rise in his creatinine levels from a pretty stead 100 to 114 . She had us do the blood work again a week later and the numbers were the same.

Today a month later, we meet with a different transplant doctor than we had been with. I ask him about Keith's level, he wasn't concerned. He said it won't stop the collection. It's ok. The nurse gave us Keith's lab reports and its says "eGFR results consistent with moderate CKD Stage 3."

He had no kidney problems. So what does this mean?

I googled CKD stage 3 and found some good info.

CKD stands for chronic kidney disease, Stage 3. Here is what I got:

"A person with stage 3 chronic kidney disease (CKD) has moderate kidney damage. This stage is broken up into two: a decrease in glomerular filtration rate (GFR) for Stage 3A is 45-59 mL/min and a decrease in GFR for Stage 3B is 30-44 mL/min. As kidney function declines waste products can build up in the blood causing a condition known as “uremia.” In stage 3 a person is more likely to develop complications of kidney disease such as high blood pressure, anemia (a shortage of red blood cells) and/or early bone disease"

Simply put, this condition means that his eGFR number highlights a chronic kidney disease at stage 3. This can be (does not mean IT HAS TO BE) associated with some increased chal­lenges, but your transplant doctor seems to be telling you that, despite this, they can move forward with collection.

What are the units of measure for the creatinine levels you quoted?

What was the eGFR value that was reported in the lab?

eGFR is simply a number that is derived from the creatinine level, age, sex and race.

eGFR is reported in units of ml/min/1.73m2

When I asked yesterday I was wondering about kidney damage , if he had any, but the Dr. just evaded my question by saying it won't stop transplant. So reading that on the same page he had worried me.

But what was the actual eGFR number?

Also, what were the creatinine numbers (including their units of measure)?

Creatinine levels 114 umol/L

A typical reference range for creatinine is about 60-100 mmol/L for men (you should check what the reference range is on your particular lab printout).

I'm going to guess that the transplant doc didn't see a huge issue with a value of 114 mmol/L and that the doc probably also looked at some of the other renal markers like BUN, albumin, creatinine clearance, etc when making that call. But I'm not a doc by any stretch of the imagination, so any interpretation should really come from the doctor.

To calculate eGFR values and how they correspond to CKD stage, you can plug in the 114 mmol/L value, age, sex and race in this calculator here to see how the results come out:

(note that the calculator clearly states that the eGFR from a lab report is probably more accurate than the results produced on this calculator).

Impared kidney (renal) function is one of the symptoms of active multiple myeloma. It is the "R" in the "C.R.A.B." test for active multiple myeloma. The SCT is designed to address the multiple myeloma to drive it down. If successful, he should experience improved renal function.
