What do my latest free light chain results mean?

My kappa serum is now 40.3 and my free kappa lambda is 12.21.

I had a stem cell transplant in February 2014 and I am concerned about this kappa. What does this mean?

For a FLC Assay the normal test readings are as follows:

Serum Kappa 3.3-19.4 mg/L
Serum Lamda 5.7-26.3 mg/L
Kappa/Kamda ratio 0.26-1.65 mg/L

Results are considered abnormal if they fall outside of the normal ranges

The FLC Assay should be used alongside of SPEP (Serum Protein Electrophoresis) and sIFE (serum Immunofixation) so as to tell the type of immuglobulin that is in excess and measurement of any M spike.

Hi Ron,

I have a question about the (serum free light values) you quote. I am a patient at UAMS. When they e-mail me my results the values are different from the ones you quote.

For example
kappa free light chain 0.33-1.94 mg/dl 28.10
IgG lambda free light chain 0.57-2.63 mg/dl 1.52

Anyway, is this something different than or just a different way of rating the same thing?

Marvin,

The only difference is that UAMS is using mg per deciliter (one tenth of a liter), rather than mg per liter. Thus the normal ranges from UAMS are lower by a factor of 10 than the normal ranges I listed. Otherwise they are the same.

In your case, the kappa levels are high. Your ratio is also high, as the ratio is simply dividing the kappa by the lambda levels. In your case the ratio would be 28.10 divided by 1.52, or 18.48, which is high. That indicates that you are overproducing kappa light chains and likely have IgG kappa multiple myeloma.

Marvin, I'm a kappa problem just like you. Besides watching your kappa/lamba numbers, you should also monitor your IgG numbers. My numbers are similar to yours and have remained steady for the last 16 months.

My IgG numbers fall in the acceptable range. Thanks for pointing them out. I'll watch them closer
now.
