What could be causing these symptoms?

With multiple myeloma, what could be causing gout in hands with normal uric acid, retaining fluid and extreme tiredness. My husband has had this twice in one month.

Thank you for any insight on this issue.

During treatment there is cancer cell killing that can lead to increases in uric acid (even though levels not out of reference range) that can precipitate gout. This is not uncommon.

And allopurinol can help prevent this. But not to be started during acute flare of gout.
