Questions and discussion about monoclonal gammopathy of undetermined significance (i.e., diagnosis, risk of progression, living with the disease, etc.)

What types of cancer can MGUS turn into?

by countrygirl on Tue Dec 06, 2016 7:31 pm

So I'm having a hard time pinning this down. I've gotten different answers. Do all types of MGUS turn into myeloma only, or do some types, like IgG, only turn into myeloma, and non IgG types into myeloma or lymphoma or leukemia?

Does this help?

"Monoclonal gammopathy of undetermined significance (MGUS) is a common premalignant plasma cell disorder that can be detected in more than 4% of individuals older than 50 years. MGUS is classified into three distinct subtypes: immunoglobulin-M MGUS, non-immunoglobulin-M MGUS, and light-chain MGUS. Risk of progression of non-immunoglobulin-M MGUS to multiple myeloma has been well characterised. By contrast, immunoglobulin-M MGUS rarely progresses to multiple myeloma, but rather progresses to Waldenström macroglobulinaemia."

Thanks for the info. Would you please also explain how do we know what kind of MGUS we have?

Hello,

Type of MGUS is determined from the immunofixation electrophoresis blood test, which tells you whether you have, for example, monoclonal immunoglobulin G (IgG) kappa in your blood, or IgA lambda, etc.

But, already in your quantitative immunoglobulin test results, you have some hints. Normally, the type of immunoglobulin that has a monoclonal fraction is elevated, while the others are suppressed.

So, for example, when you have (like me) immunoglobulin A (IgA) MGUS, the IgA level in the quantitative immunoglobulin results is elevated, and IgM and IgG are low or very low.

But, final classification can be only done with the serum immunofixation.

Hope, this helps,

Johannes

Thanks Johannes for your reply.

I have IgG MGUS and my serum immunofixation are in range, then I think I have immunoglobulin IgG MGUS! This disease is very complicated. My IgA is in a normal range, but my IgM and IgG is in high range! My very myeloma specialist said he has no idea why my IgM is also high! I did the bone marrow biopsy and is confirmed that I have IgG MGUS.
