Understanding our test results

by cgrundy on Sun Dec 29, 2013 10:51 am

My husband Ken has a stem cell transplant in January 9, 2013 and he had 3 months of undetected MP until last month when his MP was 46.8 and this month it jumped to 102. We are so nervous that he had a relapse. This is what the results stated:

Details
Past Results
Graph of Past Results
Component Results
Component Standard Range Your Value Flag
IMMUNOGLOBULIN G (AKA IGG, NEPHELOMETRY) 600 - 1500 mg/dL 1110.0
IMMUNOGLOBULIN A (AKA IGA, NEPHELOMETRY) 82 - 453 mg/dL 377.0
IMMUNOGLOBULIN M (AKA IGM, NEPHELOMETRY) 46 - 304 mg/dL 64.3
TOTAL PROTEIN-TP 6.4 - 8.3 g/dL 6.8
ALBUMIN 3.7 - 4.9 g/dL 3.6 L
ALPHA 1 0.2 - 0.4 g/dL 0.3
ALPHA 2 0.5 - 1.0 g/dL 0.8
BETA 0.5 - 1.1 g/dL 0.9
GAMMA 0.6 - 1.5 g/dL 1.2

SPE INTERPRETATION There is a zone of restriction in the gamma region with a normal remaining polyclonal immunoglobulins. This pattern suggests monoclonal or oligoclonal proteins that may occur in chronic inflammatory diseases (e.g., of liver), or may be idiopathic, or may represent a monoclonal gammopathy. Suggest:

a) Quant Monoclonal Protein Immunofixation, serum which also includes the quantitative immunoglobulins.

b) Quant Monoclonal Protein Immunofixation,24 hr urine

The albumin is decreased. This may occur in protein loss, malnutrition, liver disease, and (or) as an acute-phase reactant.

SPE REVIEWED BY #2: Elizabeth A. Biller, M.D.

MONOCLONAL PROTEIN, SERUM 0 mg/dL 102.0 H

IMMUNOFIXATION, SERUM IgG lambda monoclonal protein is present.

Can someone help us understand this?

Thank you.

Cheryl

From what I understand the test results look fairly good. The SPE interpretation should have a number that is the m-spike. If it is the 102.0, then that is high, but I looks like it might be the total IgG number. My m-spike number appears in the sentence in your husband's results that are the IMMUNOFIXATION, SERUM.

Make sure that you ask your oncologist to interpret the results for you. And, ask him/her what part of the results tell you the m-spike. Also, does your husband get a free lite test done?
