Hi Everyone,
I'm 45-year-old male and concerned since results were abnormal, as follows:
Protein, Total 8.0 g/dL
Albumin 4.8 g/dL
Alpha-1-Globulins 0.2 g/dL
Alpha-2-Globulins 0.5 g/dL
Beta Globulins 1.1 g/dL
Gamma Globulins 1.4 g/dL
Kappa 329 mg/dL
Lambda 184 mg/dL
Kappa/Lambda Ratio 1.8
Immunoglobulin A 262 mg/dL
Immunoglobulin G 1364 mg/dL
Immunoglobulin M 59 mg/dL
Immunofixation, Serum:
Reference Range: No monoclonal proteins detected
Immunofixation studies revealed ONE trace concentration band migrating
in the GAMMA region which was of too low a concentration to definitively
identify.
My neurologist ordered this study since I was diagnosed with polyneuropathy and muscle denervation in the tibialis anterior, also radiculopathy.
I'm totally lost and concerned. Please your guidance and orientation, It will be much appreciated.

Were these from a urine test or a blood test? If a urine test, was it a 24-hour urine test or a random (one-time) sample?
Also, for the kappa, lambda, and ratio results, are those for total light chains or free light chains?

This assay provides a measurement of the total kappa and the total lambda light chains, and it was done in blood, thanks

OK. And the other results, such as the immunoglobulin levels and immunofixation results -- those were from a blood (serum) test as well?

Some of your numbers are elevated, but others not.
I guess no M Spike --with these numbers, means No active myeloma.
Albumin is good, ratio is less than 2.65, etc.
Could your neurological emanate from trauma, a wasting disease, kidney disease,.....or say something like Diabetes (Pre-onset) ?

My neurologist didn't answer my calls. Neither my PCP. Tthat's why I'm worried. I also had C-Reactive Protein, hemoglobin A1C, Vitamin B12 / Folate in the same blood test, all came back normal. I also had a std blood test one month ago with CBC, also normal:

Hemoglobin 15.1 G/DL
Hematocrit 44.9%
MCV 92.8 fL
MCH 31.2PG
MCHC 33.6 G/DL
RDW 13.5%
Neutrophils 57%
Lymphocytes 30%
Monocytes 10%
Eosinophils 3%
Basophils 0%
Platelet Count 246 K/UL
Please your comments and what should I do next.

Adding to what Rneb said, you have no M-spike; and the symptoms that you state are not any of the "CRAB" symptoms (you can quickly look that up), so unlikely it's myeloma. This disease, however, is very complicated, and you can rarely rule something in our out 100%. Your kappa lambda ratio is sort of OK. As I recall, regarding kappa/lambda, one or the other (but not both) out of whack could be a myeloma marker. Of interest with your numbers, however, is that both are very high. I would pursue that with your doctor. I have no idea what that might be, but based on "myeloma basics," it's probably not myeloma (with the emphasis on the probably). Good luck.

Thanks for your answer. Actually, the values are so high because the lab measured total light chains instead of free light chains. Therefore we cannot conclude anything with these values. The rest appears to be normal, but I'm totally new on that. Please correct me if I'm wrong.
