Re: Thyroid hormones and multiple myeloma

by Dianem on Fri Sep 26, 2014 10:30 pm

Hi Multibilly,

I believe when I was younger, my levels were always in the average range, but once I hit the 50s, it appears the T4-T5 started getting higher and the THS lower. Yes, I slowly reduced the dosage (with doctor approval), and now all levels are in the middle range.

I have 7 'cold' nodules assessed every 4-5 years on the non-functioning thyroid to see if they have changed. In a couple of weeks I'm having an ultrasound to check the nodules. I'm willing to reduce dose lower if my doctor supports it. I feel in my case, the reduced need is related to age.

Have not noticed any energy or fatigue issues. I live in Phoenix and hike almost everyday. It's worth considering. Less is more! Diane

Hey Spanish Mary,

Regarding "...I did let my integrative doctor know what I am doing and I can only hope she understands why, as I also forwarded the Cohen report to her....", good luck to you. I will be curious to see if your markers change.

I have been taking thyroid medicine for many years. I was told by a dietician that for my test results I should be at a higher dosage. Now I am not sure what to do.

Hi Carolee,

It is always safest to have your hormone levels and medication supervised by a medical doctor.

Our mental state and immune systems seem to forever be entangled in a delicate duet, so don't burden them with unnecessary worry.

Take your results to a doctor you trust and let him/her guide you :-)

I agree with Spanish Mary regarding her suggestion to Carolee. Let the MD advise you and don't let a dietician get under your skin (nothing against dieticians – they provide an invaluable service – but they are not MDs, and treating a thyroid condition can be tricky business).

I recently was diagnosed with MGUS and have been on Synthroid for several years. I take 100 mcg, I have Hashimoto's, and am interested in learning more about the connection between the two. My physician didn't think there was any connection between the two or with the medication. Any information would be greatly appreciated.

Since the latest Beacon poll ("Weekly Poll – Hypothyroidism & Multiple Myeloma") has piqued my interest in this topic, I thought folks might find this 2008 paper abstract to be interesting:

M Dalamaga et al, "Association of thyroid disease and thyroid autoimmunity with multiple myeloma risk: a case-control study," Leukemia & Lymphoma, 2008, Vol. 49, No. 8 , Pages 1545-1552.

"...Controlling for the above variables, an individual suffering from any thyroid disease more than 10 years has about 2.41 times more likely the risk to develop multiple myeloma than an individual without any thyroid disease"

Full Abstract:

Thyroid disease has been associated with lymphohematopoietic cancer (LHC). No previous study using clinical, sonographic and laboratory data has explored whether thyroid disease and specifically autoimmune thyroid disease (ATD) is associated with multiple myeloma (multiple myeloma) risk.

73 patients with incident primary multiple myeloma and 73 hospital controls admitted for non-neoplastic and non-infectious conditions, matched on gender and age were studied between 2001 and 2007. Blood samples were collected. All subjects were submitted to clinical, ultrasound and laboratory thyroid evaluation.

The prevalence of clinical thyroid disease in multiple myeloma patients was significantly higher than in controls (p = 0.002). ATD was associated with increased risk of multiple myeloma, adjusting for age, gender, body mass index and familial history of LHC [OR = 5.68, 95% confidence interval (CI): 1.69–19.13].

Controlling for the above variables, an individual suffering from any thyroid disease more than 10 years has about 2.41 times more likely the risk to develop multiple myeloma than an individual without any thyroid disease (OR = 2.41, 95% CI: 1.35–4.29). Also, adjusting for age, gender, BMI and family history of LHC, a familial history of thyroid disease is associated with increased risk of multiple myeloma (OR = 3.23, 95% CI: 1.25–8.31).

Further studies are needed to explore underlying mechanisms associating thyroid autoimmunity with plasma cell transformation.

This is a report back on my results after I (unilaterally) decided to take a quarter of my Diotroxin dosage to see if it would have any effect on my markers. It did not and after seeing my integrative doctor today, I am resuming the normal dose.

The results:
M spike 1.4 g/dL [14 g/L] (basically unchanged)
Lambda light chains 66 mg/L (up from 52)
Ratio 0,19

I also saw my hematologist this week, and she confirmed what she has mentioned to me before, that in her experience it is usually IgG lambda that progresses. She considers aggressive treatment when the M-spike reaches 2.0 g/dL [20g/L] to minimize kidney damage.

Is this a practice followed anywhere else?

I have never heard of any doc recommending initiating treatment based solely on reaching an M-Spike of 2.0 g/dL. My M-spike has been in the range of 2 g/dL for over two years now and my FLC ratio is 0.05. I also had a kidney biopsy which showed that I have zero renal damage. And all my kidney serum markers also back this up. I have seen 4 different specialists and not a single one suggested that I even consider treatment at this time.

For the most part, it's usually the CRAB-I symptoms or the additional new criteria mentioned here that one uses to initiate treatment.

SV Rajkumar, "New Criteria For The Diagnosis Of Multiple Myeloma And Related Disorders," The Myeloma Beacon, Oct 26, 2014.

Are your serum renal markers (BUN, Creatinine, etc) currently of concern?

My glomerular filtration rate is at the bottom of normal and creatinine high but in spec. Our units are different from yours and my results as follows (with spec range in brackets)

Sodium 141mmol/l (136 - 145)
Potassium 4.6 mmol/L (3.5 - 5.1)
Chloride 104 mmol/L (98 - 107)
Bicarbonate 26 mmol/L (22 - 29)
Urea 6.9 mmol/L (1.7 - 8.3)
Creatinine 82 umol/L (49 - 90)
eGFR (MDRD) 62 ml/min (62 - 106)
Ca corrected 2.49 mmol/L (2.15 - 2.50)

UPEP and urine immune fixation : no monoclonal free light chains and 0.04g/L urine protein
I am still neutropenic 1.38 (2.0 - 7.5)
My vit B12 levels are good : 654 pmol/L (156 - 698)

The confounding number is the BMB WITH 0.02 % clonal cells (done in June 2014). It could be a sampling problem
