Should I have more tests to rule out MGUS or myeloma?

Hello friends!

Being quite the "professional" forum user :), I must admit that I have never seen such a helpful, knowledgeable, and amazing bunch of people as I see here, it must be very encouraging for many to meet you all here.

I am Alex, 42 years old from Canada.

For the last 4 years, I was suffering from thoracic (mid back) pain, and some gastro-related issues. I have had many tests done, such as abdominal CAT scan, abdominal MRI, HIDA, and even gallium scan. All tests were negative. All blood tests were also negative with CBC on the low side of normal.

About a year ago had pretty bad back pain with fever, went to my GP and she ordered an x-ray of the thoracic spine that showed slight wedging of T8 and T9 as well some generative spurring.

As a result, I was referred to have a bone density scan that diagnosed significant osteopenia.
I was referred to an endocrinologist who ran a bunch of blood tests and all the results were in the normal range except Anion Gap that was 4 with normal range of 8-16 mmol/L and also low vitamin D. Total protein, albumin, creatinine, calcium were in normal range. The endocrinologist said that he cannot see anything wrong.

About a month ago, I started to feel weird "carpal tunnel syndrome" sensations, tingling and restless legs, looks like something related to the neuropathic domain. At the same time about a few months ago, I went to see a naturopath, had a very long discussion with her and sent her all lab results I had from all last years. A few days ago, she asked to follow up with my GP and suggested to exclude multiple myeloma.

So I went back to my GP and asked for the tests.
Today I got the results but not sure if they show anything related to multiple myeloma.

Since all these tests are in the normal range, is there anything else I should ask my GP to test, or that would be enough to rule out MGUS or multiple myeloma?

Just got results for the SPEP

Albumin 48.7 36.0-51.0 g/L
Alpha 1 Globulin 2.2 2.0-4.0 g/L
Alpha 2 Globulin 5.9 5.0-9.0 g/L
Beta 1 Globulin 4.2 3.0-6.0 g/L
Beta 2 Globulin 3.8 2.0-5.0 g/L
Gamma Globulin 12.2 6.0-16.0 g/L

Serum protein electrophoresis does not suggest a monoclonal pattern.


Do I need to ask for any other tests or that's enough and we are back to the square one?

Hi Alex,

The SPEP result saying that the test "does not suggest a monoclonal pattern" is a good result, in that it is one piece of evidence suggesting that you do not have MGUS or multiple myeloma.

However, the result does not completely rule out MGUS or multiple myeloma.

Did you have a serum immunofixation test and/or serum free light chain test done?

Having these tests done would go a long way to ruling out the possibility of MGUS or multiple myeloma. Some 15-20 percent of multiple myeloma patients have light chain-only disease, meaning that SPEP tests show no monoclonal pattern. For these patients, you can only monitor the disease and responses to treatment with the serum free light chain test.

A few weeks back I was back to the same endocrinologist to repeat the bone density and eval­u­a­tion of my T8-T9 compression fracture.

He ordered a few tests and confirmed that osteopenia is getting worse and the result is border­line with osteoporosis. Also, he could clearly see compression fractures at T8 and T9

He could not pinpoint the reason for osteoporosis but ordered SPEP and serum immunofixation. The results were:

Total Protein Serum 71 60-80 g/L
Albumin 48.0 39.0-51.0 g/L
Alpha 1 Globulin 1.6 1.0-2.3 g/L
Alpha 2 Globulin 6.0 5.0-10.0 g/L
Beta Globulin 6.0 4.0-11.5 g/L
Gamma Globulin 9.3 5.0-13.0 g/L


Serum interpretation: Query faint band in gamma region
Serum immunofixation: Band in IgG kappa, but not distinct enough to call it monoclonal protein.


The question is what should I do in this case? Would it make sense to ask for a referral to a hematologist-oncologist? Should I ask for any other tests to rule out MGUS or multiple myeloma?
