The comment below from Dr. Cohen in this thread might help you regarding the osteopenia question with respect to CRAB (note the word "severe" in the response). But please be clear that women (and men) can develop osteopenia for a variety of different reasons other than menopause or bone loss/resorption due to multiple myeloma. This would be another good subject to discuss with your docs.

"Lytic bone lesions visible on X-rays or other radiologic tests reflect active myeloma and are an indication for treatment. Severe osteopenia or osteoporosis also is technically included under "B" in the CRAB criteria. However, unless there is a significant history of fractures, I do not usually initiate anti-myeloma therapy for osteopenia alone, and would concur with just Zometa and close observation.
This is an individualized decision that should be made by each patient and his/her oncologist.
