I would recommend that you read the book Being Mortal. It is written by a doctor who had cancer and the same questions we all have. The insights there will help you make the right decision for you.

We have a doctor just like what you are describing. He has warmed up considerably over the years and the medical care has been excellent.

Perhaps it is the same doc? I sure hope things improve with my doc. In general, I have not been feeling all that great lately and have significant back pain and associated leg pain (radiculopathy). Also have lost weight the last few months.
But as of a few weeks ago, my labs are pretty much the same and show no major changes.
