Is sudden back pain a sign the myeloma is getting worse?

Hello everyone,

My dad has been doing well for the most part up until recently. He started having some back pain last week along with some congestion and coughing up stuff. He has missed three days of work within the past week and that's not like him. He had to leave work early yesterday because he sneezed and said it hurt him so bad he could hardly move. He's still sore today and didn't go to work, but he's being stubborn and won't let me take him to the doctor. I practically begged him cause I know that sudden pain is never a good sign in a cancer patient.

Could this mean the multiple myeloma is getting worse?

His levels were good in May when he went to the doctor. He insists he's fine, but I have a feeling something's wrong. :cry:

Hello Amber:

The natural reaction for someone with multiple myeloma (and most any other type of cancer, as well) is to worry that the cancer is coming back with a vengeance at the first sign of any pain.

As your Dad crosses 50 years old, however, he also is naturally subject to the same aging process as anyone else. There are many other causes as well. My wife got the scan results for constant low level back pain a while back. The "good" news was that it was seen to be the onset of arthritis.

My wild guess is that for most instances of general back pain that crop up, go away, and maybe comes back (possibly in a slightly different place) is that they might be most likely something like a muscle or nerve, maybe arthritis, but most likely not the myeloma. He should be getting his scans periodically. That of course should be looked for onset of bone problems.

For colds and infections, as well, the same applies. A "healthy" person will get something like two to four instances per year of a cold, a flu, or related illness. A myeloma patient on treatment or in remission would be expected to get at least that amount. The immune system is com­promised to some degree, and some patients will naturally get a little bit more frequency of colds and flus (or the same frequency and a little more severe), and it does not necessarily mean that the myeloma is advancing or getting any worse.

Due to the nature of the illness, however, all these issues should be reported to the medical team.

I hope this allays your worry a bit for a while, and keep up the great work in supporting your Dad.

He is still the same, but now complains that his ribs are hurting too. The rib pain is how he was diagnosed back in 2014, so now I'm worrying more. He has an appointment coming up on August 15th, but doesn't have a full skeletal survey until October. He promised me he would tell his doctor what was going on when he goes in August. Maybe they'll do a scan then.

Hello Amber,

It's so nice of you to be looking after your Dad. Try not to worry too much. As long as your Dad is being seen regularly by his doctor if the pain is myeloma there will still be time to get it treated. The pain could be a variety of things but I know it's hard to not worry.

I go through the same thing all the time. Always wondering about a new pain I might have. Just before I was first diagnosed, I sneezed and fractured a vertebrae. It was several months later before I learned of the myeloma being the cause. I am 50 now and have pain from osteo­arthritis and muscle strains that come and go. I also have experienced severe back pain from urinary tract infections.

Just make sure he lets his doctors know of any pain he has.

Just adding my $0.02. Get a MRI. My hubby had back pain and got a full skeletal x-ray (something like 15 x-rays, $1800). Showed no lesions, we were thrilled. Still had pain. Got an MRI, showed 2 compressed discs and 2 partially compressed. Kyphoplasty fixed the pain in the compressed discs after about 2 weeks.

I appreciate the replies. I will have him tell his doctor about his pain even if I have to go up there with him. I know he's just scared because he told my cousin he didn't want the doctors to tell him that he's getting worse or the treatment isn't working anymore. I can't blame him. I will also suggest that he should get an MRI done.

My mother also had back pain and we thought it was a muscle pain because initially she com­plained pain on her left side and then on the right side. The x-rays did not show anything and the doctor said it might have been due to age (she is 62). The pain then kept on increasing which led to MRI that detected she had compression fracture. She was given radiation and Revlimid, Velcade, and dexamethasone.

I would definitely suggest an MRI as in our case this was the second time things didn't show up in x-ray. Initially also she was treated for frozen shoulder and after a pathological fracture was diag­nosed with plasmacytoma.

I had a radiologist tell me recently that a significant number of people with myeloma come to him with issues that are completely unrelated to multiple myeloma, and are simply degenerational issues that affect everyone.

Don't worry too much, if the myeloma is active enough to cause the problems your father is experiencing, it'll show up in the labs. That's one of the few nice things about multiple myeloma, it's relatively easy to monitor.

Hi Amber,

Please call the oncologist office and find out how you become one of the people your dad has approved for his doctors to share and discuss his medical treatments and myeloma issues. This is not the same as being an emergency contact.

Also, anytime you have a concern, and this is a big one, you can call their office and request that your message / concern be passed on to the oncologist. You can do that without yet being a personal representative. Sounds like you are your dad's advocate? If so, it's good to make yourself known!
