Smoldering myeloma with low albumin
I have just noticed on my blood tests that my albumin levels are 3.4 g/dL. I am diagnosed as having smoldering multiple myeloma. Just wondering if the low level of albumin is a concern as the staging criteria notes <3.5 as indicative of Stage 2.

Hi Sandi,
I'm no expert, but I believe albumin below 35 is used to stage the disease for people who have active (symptomatic) multiple myeloma, not smoldering myeloma.
How are your other results (e.g., M-spike, etc)?

Thanks for your response. Had myself a bit worried as my albumin and beta 2 microglobulin were both within stage 2 range (and the only criteria for stage 2). Had spoken to myeloma nurse not long after my initial post and she had explained the need to meet the CRAB criteria before staging. Yes, I am still smoldering. Now anaemic, which is being monitored for 3 months (not low enough to meet CRAB criteria at this stage). Haematologist-oncologist concerned about bone marrow changes and will request another bone marrow test if anemia remains at next visit. (I think also in response to my bone density having rapid decrease in last 12 months). Raises a query for me however, that if my anaemia does end up meeting the CRAB criteria, am I automatically at stage 2 because of the albumin and beta 2 results.

Hi Sandi,
This is such a complex disease. With other cancers just you're told where it is, and how big it is. Myeloma requires taking a degree course, and preferably a Ph.D
Bone density and anaemia results could be from some other condition.
Hopefully your next results go well.

In my last analysis, 5 days ago, I had a low albumin rate (I always had a normal rate, this is the first time that it happens to me).
I have stage 1 monoclonal gammopathy (IgG kappa).
I had 36.4 g/L of albumin in the electrophoresis of proteins (normal range: 40.2 - 47.6 g/L). and I immediately asked for advice. Two doctors told me it was not very low, given that it's above 30 g/l.
My Alpha 1 and 2, and Beta 1 and 2, were a little above normal, and with low albumin a doctor suggested a small inflammation.
In recent months, having had shingles, then having had a fall, I took a lot of paracetamol (Tylenol / acetaminophen) (and the codeine made me sick). I think that my liver has been abused, and since it is the liver that handles albumin ...
And look at the figure of gamma globulins: if it is in a stable range, I would not be worried.
