Hello!
Hi Multibilly thank you for so much help.
I find something more in morphology report. Now you may be able better check my Mayo model.
"Plasma cells are increased in number on CD138 evaluation and comprise at least 20% of total bone marrow cellurarity. Lambda light chain restriction and no evidence of amyloid is confirmed with kappa-ISH, lambda-ISH, and Congo Red. CD56 is negative on the plasma cells but a dual cyclin D1/CD138 stain reveals con-expression of cyclin on the neoplastic plasma cells. PAX-5 shows concordance with CD20, highlighting a mild B-lymphocytosis of approximately 5-10%"
I post it regarding FLC ratio, I don't know if it helps.
Today I was second time in doctor office and took all the tests to looking for freelite ratio.
Thank You.

Re: Do I have smoldering myeloma or MGUS?
Well, it sounds like you will be seeing a multiple myeloma specialist soon, so he/she will undoubtedly run any missing tests like the Freelite assay and can better explain all this to you. In the meantime, I would try not get too hung up on the various reports from the bone marrow biopsy (like the one below). Some of the details are extremely complex for any but a doctor to understand.
But it sounds like you have 20% plasma cell involvement. This is also telling you that your lambda free light chain is the involved free light chain. You will also have a companion immunoglobulin (IgG, IgA, or IgD) that will be involved. You can figure out which immunoglobulin is involved by looking at which one of your immunoglobulin levels is elevated in your lab reports. It may also tell you what kind it is by looking on your SPEP where they tell you your M-Spike. More than likely, it is the IgG that is involved (this is what most people have), but your tests will tell you for sure. It's good to know the type of myeloma you have as you monitor this disease.
When you see the specialist, I'd suggest you write down all your questions beforehand and record your meeting with the specialist (or bring along a friend). It's easy to get sidetracked and/or forget half of what you heard at these meetings.
If you want to see what all the different tests are that one typically gets when being completely diagnosed for a stage of multiple myeloma, see this link:
