Are there test results that could indicate new lesions (besides PET scans) even if there is no M spike and kappa and lambda free light chains are within normal range? I'm assuming that physical pain and high calcium would be considered, but is there anything else to look for?

From your question, I'm guessing that you suspect you may be developing a lytic lesion? Instead of a PET/CT, you can always get a localized xray of the region in question. Sure, an xray may not pick up really small, developing lesions, but it is fast and pretty inexpensive.
