My mother is in CR about 6 months now but her serum free light chain ratio is always near the upper limit. What does it means? She has Kappa light chain myeloma but her lambda light chain is sometimes lower than the limit. Why is happening that? Still didn't have any transplant only 3 rounds of chemo and now is out of treatment. Could you please let me know how does it look her Light chain essays? It looks like a durable remission as her doctor says?

AUGUST - KFLC:16.5 (6.7-22.4) , LFLC : 11.3 (8.3-27) k/l : 1.46 (0.31-1.56)
SEPTEMBER - KFLC: 20.6 (6.7-22.4) , LFLC : 12.7 (8.3-27) k/l : 1.62 (0.31-1.56)
OCTOBER - KFLC:19.30 (6.7-22.4) , LFLC : 12.2 (8.3-27) k/l : 1.58 (0.31-1.56)
NOVEMBER - KFLC:10.9 (6.7-22.4) , LFLC : 7.48 (8.3-27) k/l : 1.46 (0.31-1.56)

I am not a Doctor so what I say has to be taken as just another patients reading. I have light Chain Igg Kappa multiple myeloma. I too am measured through the Light Chain Assay. The readings you posted seem to very good. The ratio has generall been within the limits. I cannot say whether or it is a durable remission or not. However, there is nothing I see that looks like things are trending bad. Keep in mind you only have 5 months worth of data. I have been in remission for over 3 1/2 years with the lamda light chain sometimes falling below the lower end but the ratio always tracking normal. When I was first diagnosed the Kappa level was way over the max with the Lamda level being near minimum.

These number look very good overall. I am assuming that the kappa light chains were elevated before she started treatment -- now they are in the normal range. Often the uninvolved light chain (in this case lambda) can stay suppressed for a long time after treatment, so the ratio may still be slightly abnormal. There also is normal fluctuation in the tests from month to month. Overall these are encouraging results.

Thanks to all of you for your ansawers. When i was diagnosed the FLCKappa was 7200 and the FLClambda was under the minimum. The cytogenetics has shown Del17p13. I has 2 cycles of chemo i achieved CR and then 4 months rexaling. Now we are thinking to continue with an ASCT.
