Hi all -
My husband has developed a faint (not measurable) second M-spike following induction therapy. I am wondering whether this would require further inquiry or whether it is assumed to be benign (e.g. MGUS) because it is so faint.
On the other hand, the IgM lambda paraprotein is different from his IgG kappa myeloma, and am not sure whether the appearance of this clone requires additional testing.

This could be what is called secondary MGUS or atypical serum immunofixation pattern. It generally appears after transplan t but with the advent of novel therapies it can occur without therapy. It is a rather involved topic so if you search those terms in the forums or or Google them you will get quite a bit of information.
