I’m just starting my 6th cycle of 14 days on and 7 days off. I get Velcade shots twice a week (Tuesdays and Fridays). I take 20 mg of dexamethasone on injection days, and the next day. Revlimid (10 mg) is daily for the 14 days. The sequence starts on Tuesday.
I haven’t experienced any notable side effects other than some short-term brain fog. The regimen seems to have moved my labs into mostly normal range, although my M-spike is still 0.2 g/dL (2 g/l).

Re: Revlimid, Velcade, and dex (RVD) 2 weeks on, 1 week off
I'm also wondering how common this kind of treatment cycle is for RVD.
It appears my husband will be on the following cycle beginning the end of July (after his 10 radiation treatments):

- Revlimid daily (1-14 out of 21)
- Velcade on days 1, 4, 8, and 11 (out of 21)
- Dexamethasone on the Velcade days

Re: Revlimid, Velcade, and dex (RVD) 2 weeks on, 1 week off
I was on the 14/7 cycle of Revlimid, Velcade, and dexamethasone (RVD) for induction and did quite well. I did have some reductions in dosage of Revlimid, from 25 mg to 20 mg, over time due to the build up of side effects. Also reduced Velcade dosage a bit to due to slight neuropathy.
However, my M-spike quickly increased from 0.38 g/dL to 1.5 g/dL during the 4+ week break in treatment before my stem cell transplant, but dropped during the first 100 days post transplant without treatment to 0.81 g/dL and dropped even more quickly during 3 cycles of consolidation with Kyprolis, 20 mg dex, and 10 mg Revlimid. One more cycle to go and then regular maintenance.
I'm hoping to do the 14/21 cycle again on maintenance with Revlimid or Revlimid and dexamethasone. Would really like to stop the dex and I think the 14/7 allows you blood counts to recover more quickly. We'll see.
Best of luck with treatment and know that whatever treatment you do, dosage can be adjusted based on reactions and/or other treatments can be used.
