Re: Recently diagnosed with MGUS (further testing underway)

Did everyone stop answering my question :(?

Anyway, I went back to the doctor and he did more testing said it's not too high right now and said that, if everything stays about what it is now, he will just monitor me every 2-4 months.

Is this normal?

sknc0604:

I'm sorry if your question was not answered clearly. I am new to this too, so I will do the best I can. Maybe someone else with more experience can chime in on the blood test levels you reported.

It looks like your immunoglobulin G, M and A are all well within the normal levels. Your beta 2 microgobulin is fine. I can't seem to get a straight answer or good understanding myself about the kappa/lambda stuff, so maybe someone else will pipe up?

I also have had increased problems with pain and neuropathy, particularly the feet and lower legs. My GP sent me to a neurologist, the neurologist sent me to the hematologist/oncologist because of an elevated M-protein. They are ascribing it to degenerative disk disease, compression of nerve root in two parts of my spine. I also have fibromyalgia, so I have pain/aches pretty much all the time.

There's another post here from someone with MGUS who asked about having a bone marrow biopsy (BMB). A physician chimed in to say that it's a good idea to have one to confirm the diagnosis of MGUS or to determine if it's SMM, smoldering multiple myeloma, instead.

God bless you and thank you so much for the sacrifice of service to our great country through your son!.

Toni, I'm sorry I just read your message because I never got a notice that I had messages and I would check to see and never saw them. But my levels the next time all went up in a week. Not dramatically, but my IgM I think it is was now above normal, barely, but above, so he is watching.

I am in severe pain all the time. Was in my feet from neuropathy, but also now in my legs, hips, and arms and shoulders. I don't know what to do anymore.

I don't go back to doctor till October but I'm in such severe pain I don't know if it is from this or what.

What kind of pain medication are they giving you? Please be sure they are at least treating the symptoms. I take gabapentin for pain and on rare occasions, Vicodin. I take another medication for something called PLMD that helps with spasms.

I'll check back later. Gotta run right now. There are a lot of pain medications out there that can help you.

They gave me gabapentin for the neuropathy but that is it. I have a spinal stimulator also in my back to help with the neuropathy, but it doesn't seem to work much anymore and now with the new pain that I am having above it, like in my arms and all, it doesn't help it anyway.

I am calling my doctor tomorrow to talk to him about this pain and see what he says. All I know is I am a very active person, always on the run, and this pain and getting tired all the time now is really starting to get to me.

Also I am near my results, they retested and gave me the results of the IgG went from 792 up to 833 with a range of 700-1600 being normal so that one was good.

My IgA went from 145 up to 155 with a range of 91-414 being normal so that one was good.

And my IgM went from 216 up to 231 with a range of 40-230 being normal, so that one went a little above the normal range.

These test were a week apart and I am supposed to go back in October for more testing and see what my levels are then he will decide then about doing the bone marrow biopsy.

Thanks for getting back to me.

It's been a while since I talked to anyone on here so I thought I would give an update.

I am going to have a full body x-ray in just a few minutes, and I am getting my bone marrow biopsy done Monday morning.

Ok, this is what the results were from my body x-rays. Can someone help me translate it please?

BONE SURVEY COMP (METS) 10/8/2014 6:26 PM
IMPRESSION: 1. Small circumscribed lucencies in the proximal right and left humerus are symmetric and may reflect normal variant or summation shadow, less likely lucent lesions of other etiology. 2. Small sclerotic foci in the proximal right and left femur with symmetric appearance may represent enostoses, less likely sclerotic lesions of other etiology.

HISTORY: MGUS.

FINDINGS: AP and lateral skull, AP chest, AP pelvis, AP and lateral lumbar and thoracic spine, AP views of both arms and forearms, PA views of both hands, AP views of both femurs and lower legs. 20 views are obtained. Postoperative changes are seen in the cervical spine. Spinal stimulator is present. Degenerative bony changes are seen. Right upper quadrant surgical clips are present. Small circumscribed lucency is seen in the proximal right humerus. Small circumscribed lucency is also seen in the proximal left humerus. There is a small sclerotic focus in the proximal metaphysis of the right femur. A similar small sclerotic focus is seen in the proximal metaphysis of the left femur. No other lytic or sclerotic bony lesions are seen. No acute fracture or dislocation are identified. Surgical clips are seen in the pelvis. Mineralization is normal

The pathologist does not ascribe the findings to MGUS/multiple myeloma and seems to think they are a normal variant or summation shadow. In other words, we all have slight differences or oddities of our bodies. But that does not mean they are attributable to illness or conditions requiring treatment.

Your lab results included these findings:

1. Small circumscribed lucencies in the proximal right and left humerus are symmetric and may reflect normal variant or summation shadow, less likely lucent lesions of other etiology.
2. Small sclerotic foci in the proximal right and left femur with symmetric appearance may represent enostoses, less likely sclerotic lesions of other etiology."

I was not familiar with the term enostosis, so I had to look it up. These are also issues that do not require treatment.

"An enostosis or bone island is a small area of compact bone within the cancellous bone. They are commonly seen as an incidental finding on radiographs or CT scans. They are typically very small and do not cause any symptoms. No treatment is necessary." - Wikipedia

Keep in mind, though, I am in no way qualified to interpret these findings (i.e., I'm not a physician). So you will need to discuss these findings with your own physician. It seems you have multiple medical / health issues, so it does require a concerted effort by your health care team.

All the best.

I forgot to add: the bone marrow biopsy will probably help greatly in determining if MGUS is the correct diagnosis in your case. It usually takes a little while for the results to come in, but I think you'll finally get some answers you are really looking for. The waiting is hard though! :)
