Can someone explain what role the ratio in the free light chain test serves?
Also, I know you can have no M spike but high light chains / abnormal ratios. I am wondering can you have normal light chains and a normal ratio but still have an M spike?

Blair77, I had normal light chains and normal ratio in the presence of an M-spike. This was true while I was smoldering. At the time I started induction treatment, my kappa light chains had risen ever so slightly. They quickly returned to normal so I even thought that the one abnormal reading may have been a mistaken lab result.

Thanks! However, my confusion continues, as my husband's light chains continue to both be elevated and really bounce all over, sometimes kappas higher, then lambda, and sometimes vice versa. There's no trend upward or downward in either light chain, but the ratio never has been out of range since he began treatment.
Also, his kidneys have never been an issue, so that doesn't explain this.
