I just thought I would pass on information on an assistance program. Some of you might have heard of this already. I just received my approval in the mail today for $12,500 of assistance through Patient Access Network Foundation (PAN Foundation).
I have Accredo Specialty Pharmacy for my Revlimid. During my last phone call with them for patient counseling, they asked if there was anything else I had questions about. I asked them about assistance for co-pay/out-of-pocket expenses. They had an insurance assistance person call me back. I was asked several question and after it was done they advised me that I should qualify but it was up to the assistance foundation.
Requirements were that my wife and I could not exceed an annual income of $78,000. Since I am on SSDI, my income is way less than when I worked. This whole process took only about 10 days. They also sent me some proof of expenditure forms that I can use for my out-of-pocket/co-pay at my oncologist's.
The money stays at the foundation and Accredo sends a request for the co-pay. I don't get billed or have to pay first then get reimbursed for Revlimid co-pay. My co-pay is $50 now but will be $550 per prescription after I go on Medicare with a part D plan.
Hope this helps some folks that are looking for help with assistance programs. I was told that if I didn't qualify for this program, they had 4 other programs available.
