Does osteoporosis meet the "B" (bone damage) part of the "CRAB" criteria for a multiple myeloma diagnosis, where CRAB stands for
C = elevated Calcium levels in the blood
R = Renal insufficiency (kidney damage)
A = Anemia
B = Bone damage?

Osteoporosis OR the presence of lytic lesions used to be the criteria to meet the "B" in the CRAB requirements . However, the latest 2015 IMWG criteria no longer recommends the presence of osteoporosis in the absence of lytic lesions as being sufficient evidence of bone disease (the "B" in CRAB) for the purposes of a symptomatic multiple myeloma diagnosis.

I had an appointment last Thursday with a myeloma specialist, which went well. He said most probably I don't have multiple myeloma, but if I want to rule it out I must do the bone marrow biopsy. He also ordered a bone density test, which I did it on Friday, and I was told by the radiologist I have osteoporosis and that made me worried. I did the whole body skeleton X-ray and that was clear. He ran lots of blood work and I will see him on Thursday to go over the results, I am very scared of doing the bone marrow biopsy, but most probably I will do it to get it over with.
I appreciate your quick response as usual.

If the bone marrow biopsy worries you, call in advance to get Ativan (lorazepam) and a narcotic. I have done 6 bone marrow biopsies, both with and without drugs, and I prefer with. Just make sure to get a ride home.

I did the bone marrow biopsy this morning, and it was much better than what I though it would be. I took your advice, Arizonan, and thank, thanks, thanks. It did help a lot. Just discomfort, not lots of pain.

Osteoporosis / osteopenia can come from multiple myeloma. For sure in my case anyway. But I'm still in my 30's, so osteoporosis in a 30-something is pretty rare without a cause like myeloma. I had lytic lesions though. so guess that fits.
