Does oligoclonal gammopathy mean no CR or sCR?

by mikeb on Fri Jul 18, 2014 8:47 pm

Does oligoclonal gammopathy visible by immunofixation "disqualify" a person from CR or sCR, assuming other tests meet the criteria for CR or sCR? I know monoclonal gammopathy would disqualify, of course, but not sure about oligoclonal.

By my reading of the International Uniform Response Criteria document, the answer would be yes it does disqualify, but I'm not sure whether to trust my interpretation.

Dear Mike,

Oligoclonal does not exclude from CR or sCR.

Dr. Shah,
Thanks for the quick answer. That's a help and is good to know.

dnalex.
In this case (mine), yes it is after one SCT. The faint oligoclonal gammopathy just started showing up on my most recent serum immunofixation test, 14 months after my SCT. I've had VRD consolidation therapy and Revlimid maintenance therapy since the SCT. But my question really applies more generally to the response after any therapy, not just SCT.

Hello Mike,

My husband had his autologous stem cell transplant in December 2016. He is diagnosed with POEMS (osteoscleroting myeloma) in September 2016 .

His serum protein electrophoresis is also showing oligoclonal gammopathy. But his oncologists at Sloan-Kettering are saying it is nothing to worry about.

Best Wishes,
Syeda R
