At diagnosis my husband's bone marrow biopsy (BMB) was tested with FISH which showed no cytogenetic abnormalities related to myeloma. However, post ASCT he has no M spike, less then 5% plasma with a polyclonal pattern, and the only test they did on his bone marrow was a karyotype.
Why would they have not performed the FISH?

Given the very good bone marrow results, in addition to the lack of known myeloma cytogenetic abnormalities at diagnosis, I don't think there was compelling reason to do FISH again.
Bone marrow cells comprising of about 5% polyclonal plasma cells means that there are very few (if any?) myeloma cells in that particular sample, which is a very good thing.
I suspect that eventually, your doctor will add minimal residual disease (MRD) to the panel of things to be tested in bone marrow.

You cannot do a FISH cytogenetic test if there are no myeloma cells.

They ordered a FISH test on my bone marrow biopsy after my SCT, I had not had one yet. As Dr. Andrew said, the could not do it, there were not any or enough myeloma cells in the sample to perform the test. I would call this a high qulaity problem
. On my last biopsy a few months ago there were enough cells for FISH and I did get results which were pretty good, no high risk abnormalties were found.
