Neuropathy and IgM monoclonal antibody and Bence Jones

by monotreme8 on Thu Jul 18, 2013 8:51 pm

Hello. I am a new member to this forum. My husband was found to have a IgM monoclonal antibody in 2007. He was also diagnosed with severe spine osteoporosis ( T score -3.8 was one of the readings on the bone density test). At the same time, he was also developing numbness and tingling in his feet and the doctors started treating his neuropathy with drugs. Last year, a doctor ordered a urine electrophoresis and Bence Jones Proteins were found in the urine, ( alpha1, alpha2, Beta and Gamma) and the Beta Glycoprotein IGM AB was 2 in the urine and Beta Glycoprotein in the blood was 12.3.

My question is : the doctors ( neurologist who is treating the neuropathy, and the family doctor) both say that there is nothing to worry about and do not think there is any need for follow up. He does have three lung nodules but they are small and also docs think they are innocent nodules.

Should we seek an opinion of a hematologist and see if my husband should be monitored as far as Bence Jones proteins or the high Hematocrit getting worse? ( Last HCT was 55). I am concerned that when I research IGM monoclonal gammopathy and neuropathy it doesn't sound good.

Thank you for any input.

IgM monoclonal gammopathies can cause neuropathy, so if there's not another obvious explanation for the neuropathy (e.g. diabetes), it's worth an evaluation by a hematolgist to fully characterize the IgM gammopathy and see if it might be contributing. If it's thought to potentially be the cause, there are treatments available, such as rituximab, that might help. Bence Jones proteins are often seen in the urine with monoclonal gammopathies are their presence does not necessarily mean there's a problem, as long as kidney function is okay. Anti-beta-2-glycoprotein IgM antibodies are part of the anti-phospholipid antibody family and are usually tested for when there is concern about a tendency to form blood clots. They are not related to monoclonal gammopathies or neuropathy and I'm not sure why they would have been sent in this case. Perhaps they meant to test for beta-2-microglobulin?
Hope this helps.
