Need help with lab results

by bad blood on Tue Sep 29, 2015 9:15 am

Hello,
I had my mgus labs and wanted your help on the double m-spikes. All & all..my labs look good still. I will have mgus for 10 years this January. Thanks

Serum immunofixation electrophoresis reveals a doublet M-spike of
monoclonal IgG lambda in the slow gamma region.

Kappa Free Light Chains QNT 7.83 3.30-19.40 mg/L
Lambda Free Light Chains QNT 12.80 5.71-26.30 mg/L
Free Kappa/Lambda Ratio 0.61Com

Total Protein 7.4 6.4-8.2 gm/dL
Alb 5.03 4.00-5.30 gm/dL
Alpha 1 0.18 0.10-0.30 gm/dL
Alpha 2 0.73 0.50-1.00 gm/dL
Beta 0.73 0.60-1.10 gm/dL
Gamma 0.74 0.60-1.60 gm/dL
Abnormal Protein Band 0.4 gm/dL
Albumin % 68.0 %
Alpha 1 % 2.4 %
Alpha 2 % 9.8 %
Beta Percent 9.8 %
Gamma Globulin % 10.0 %
Abnormal Protein Band % 5.4 %
SPE Interp. see noteComment:
SPE reveals a doublet M-spike of monoclonal IgG lambda in the slow gamma
region (see serum IFE). Both bands, measured together, are 0.4 gm/dL. The
remainder of gamma globulin is decreased.

My next appointment is in January 2016. That is why I am asking for some help with these labs!

Igg 906 700-1600 mg/dL
IGA 153 70-400 mg/dL
Igm 73 40-230

As far as I know there is no prognosis value in having 2, 3 m-spikes vs. one, if this is indeed your question. Your numbers look good and I take it you have been pretty stable for 10 years. If there is no CRAB symptoms (calcium, creatinine, anemia and bone lesions) then you still have MGUS...

Sorry, but what help do you exactly need? "Need help with my labs" as a discussion title doesn't really make clear what your question is, and "wanted your help on the double m-spikes" doesn't de-muddy the waters much ... at least not for me.

Thank you for your reply. They didn't run the metabolic panel so I don't know if my calcium is elevated. My bone survey said diffuse osteopenia. Elevated WBC, as always, without any infection.

My mchc is low now. My bones crunch when I roll on my side at night. Leg weakness is strong but I walk a lot of miles a week to build strength. I also have bad shoulder pain. Nothing showed up on bone survey.

Yes, I'm always wondering why I have had 2 M-spikes for a few years now. Same M-spike. I have not had any treatments.

They are now running the von willebrand panel to see what type of acquired von willebrand I actually got from my MGUS. The doctor thought chemo might fix my acquired von willebrand.

Take care.
