Need help filling out SSA-3369-bk

I am filling SSA-3369-bk for SSDI. Anybody have any idea what may delay or cause me issues with SS. I am going in for stem cell transplant in December. I know the form is help to determine how disabled I am.

Anyone who has information to help complete this let me know.

I just finished filling mine out......and well I'm being told that I won't be able to collect til the actual transplant, but thats whst we want right :-) so fill it out based on now. They will request your medical records to conf. Everything anyways

My husband applied for his SSA disability application about a month ago. He just received 8 more pages of questions to answer. The cover letter included this line, "To receive SSI or Social Security disability benefits, a person must have a physical or mental impaiarment that prevents ANY work for at least 12 months in a row or that is expected to result in death".
My husband is still in the hospital (day 18 post transplant). He's sure not ready to go back to work yet, but at this stage in the game I don't expect that he wouldn't be able to go back to work within that 12 month period. How would one predict such a thing?

Be sure to include any medications you are currently receiving, especially if by induction therapy (zometa, etc.) and frequency of inductions. Put down any side effects from medications you are currently on, such as insomnia/tremors, etc from steroids, sleepiness/forgetfullness/confusion from pain killers, compromised immune system, bone pain from disease, etc. Give your doctor a heads up that you are filing claim so he/she will know to be looking for doctor forms. Followup with them to be sure they completed them and sent them back in Make sure to list all doctors and recent treatment, whether related to disease directly or not.

I actually didn't apply for my SSA disability until I was already scheduled for SCT, but turnaround time was very quick, and I had no trouble receiving payments for me and my minor children until I returned to work (over two years later; actually had two SCTs). During trial work period, I was able to continue to receive benefits for several more months. I was diagnosed in 2005, which at the time, the prognosis for multiple myeloma was not as hopeful as it is today.

As a former social security employee, I can tell you most important thing is to be as thorough as possible filling out first set of forms. Providing too much info much better than not enough. Make sure you have all contact information for health care providers, dates of hospital stays accurate, respond as quickly as possible to follow up requests. I suggest finding someone to keep up with all this for you as it can be overwhelming. Many health care providers have social workers on staff that could help.
