Multiple myeloma with kidney failture
pls help me as my father is suffering from multiple myeloma. he is in need to do the bone marrow test but for that his creatin clearance has to be 60 -70 . but he has only 38 at present. pls help me. what to do to increase the creatin clearance rate to required level

By the bone test I assume you mean a bone marrow biopsy. There are no limitations I know of that would prevent a bone marrow biopsy with a creatinine clearance level of 38. There are two basic types of creatinine tests, the first being the blood serum test and the second the 24/hr urine tests.The creatinine clearance number is a calculated value determined by the creatinine serum (or urine) value and other factors. The creatinine clearance has a inverse relationship to the creatinine level. To increase creatinine clearance levels the serum creatinine must be reduced. You have not stated what your fathers creatinine levels are. A low creatinine clearance (or e GFR)is usually a sign of Chronic Kidney Disease (CKD). One creatinine test alone is not adequate for a kidney diagnosis as there are extenuating circumstances, such as urinary tract infections, etc. which can temporarily boost serum creatinine levels.

i am sorry , i mean bone marrow transplantation for which creatinine clearance has to be 60-70. His current level is only 38. creatinine is 1.8. What has to be done to improve the creatine clearance level so that my father is eligibe for transplantation. Please help sir.

I see now why the doctors are seeking kidney improvement prior to a stem cell transplant.
Since your father has been diagnosed with Multiple Myeloma can you respond to some of the following questions?
1) Has your father been treated with any Chemo. Therapy?
2) Has your father been on any Bisphosphonate drugs such as Zometa, or Aredia?
3) Has your father been taking any over the counter pain pills such as Aspirin, Aleve, Ibuprofen or others which are NSAID (anti-inflammatory drugs) which can affect the kidneys. If so STOP!
this does not incude Tyenol.
4) Does your father have hypertension (high blood pressure) and if so over what period of time?
5) Does your father have diabetes?
6) Does your father have Bence-Jones protein in the urine as discovered by the 24/hr urine test.
7) For over what period of time in months has the serum blood creatinine, BUN, and eGFR been monitored?
There are no drugs I know of to decrease creatinine levels. The use of herbel supplements is controversial. There are some things you can do. First, stay well hydrated by drinking 6-8 glasses of water per day and for certain 1-2 days pror to your creatinine test. Second, cut down on your protein diet intake.
If you could answer some of the above questions it would be helpfull.

If I'm not mistaken, initial treatment with a regimen that includes Velcade is recommended for myeloma patients who have kidney issues.
