My mother age 65, is patient of multiple myeloma, diagnosed in June 2012. First she was on thalidomide-dexamethasone. Then bortezomib-lenalidomide (Velcade-Revlimid), then thalidomide alone. I posted some initial lab results from when she was diagnosed in this discussion in the forum.
She had relapse in March 2014 with 80% plasma cells; bortezomib with cyclophosphamide and dexamethasone started. Bone marrow aspirate in August 2014 showed 1.2% plasma cells. Her LDH and beta2 microgloblin are consistently raised. Since March 2014 she is blood transfusion dependent almost every 40th day due to drop in Hb. Serum calcium was also found raised up to 11.4 mg/dl. Cytogenetics in March 2014 was normal.
Recent protein electrophoresis and kappa & lambda levels are:
Protein electrophoresis
1. Total protein 6.20 g/dl (normal 6.4-8.3)
2. Albumin 59.59% 3.69 G/dl (normal 3.2-5.5)
3. Alpha 1 2.90% 0.18 G/dl (normal 0.1-0.4)
4. Alpha 2 16.60% 1.03 G/dl (normal 0.4-1.2)
5. Beta 11.30% 0.70 G/dl (normal 0.5-1.1)
6. Gamma 9.60% 0.60 G/dl (normal 0.5-1.6)
7. A/G ratio 1.48 (normal 1.2-1.7)
Findings on report are: a very faint band is seen in gamma region. Serum immunofixation is advised to rule out any lymphoproliferative disorder.
8. Kappa light chain (serum) 3.73 g/L Normal 6.29-13.5
9. Lambda light chain (serum) 11.80 g/L Normal 3.13-7.23
What is interpretation of above mentioned reports?
It will also be a favor if you could provide your view for future line of action with ASCT option open.
