Mother died from multiple myeloma; do I have it?

Lupus and multiple myeloma labs with the following results:

Test Result (normal range)
Cancer Antigen15-31 9.5 (0.0-25.0 U/ml Roche ECLIA methodology)
Pt, 1N-HIB Screen 11.1 (9.7-12.6 seconds)
APTT, INHIB Screen 37.0 (25.0-37.0 seconds)
DRVVT 40.4 (0.0-42.9 seconds)
K/L FREE RATIO 0.65 (0.25-1.65)
Cardiolipin IGA <9 (0-11 APL/mL Less than 12 negative)
Cardiolipin AB IGG <9 (0-14 GP/mL Less than 15 negative)


Inhibitor Screen Interpretation: See report
There is no evidence of an inhibitor or lupus anticoagulant

The following were high or abnormal

Cardiolipin Screen Positive (Negative)
KAPPA, FREE 2.41 (0.33-1.94 mg/dl)
LAMBDA FREE 3.72 (0.57-2.64 mg/dl)
Cardiolipin AB IGM 27 (0-12 MPL/mL)


The doctor told my husband and me that

1) Multiple myeloma it is not hereditary and
2) I do not have it.

However, I requested my medical records, and this is what was written:

"She has increased both kappa and lambda free light chain without normal ratio in the absence of renal insufficiency." And that he wants to redo the tests in 1 year.

From what I have read, these elevated levels are indicators of multiple myeloma. I also asked if he would like me to get my kidneys checked as I was passing stones (again) at the time I was taking the test. It's been 1 1/2 weeks with no reply. (At the time, the lab called to ask his office about kidney test, but they said to call my G.P. that they would not request a kidney test at the same time as it is not what they were looking at/for).

My mother suffered with this with 7 1/2 years and died 5 years ago. Needless to say, my family is highly anxious about these results and the lack of clarity we got from the doctor.

So question, yes I should worry, no I should not, or should I get a second opinion and my kidneys checked?

Both your kappa and lambda free light chain numbers are elevated, while you have a normal kappa/lambda ratio of 0.65. The suggests a polcyconal immuoglobluin issue and/or renal impairment, NOT a monoclonal immunoglobulin issue such as multiple myeloma. Polyclonal immunoglobulin increases can be due to infections, etc.

In any case, a second opinion never hurts. Also, chances are that they ran a comprehensive metabolic panel already. You can get a rough idea of whether you might have some kidney function issues by looking at the creatinine, BUN and eGFR readings on that test and seeing if they are all within range.

So a second test was run 3 weeks ago. Numbers have changed. Doctor is now running another more specific test, and I should have the results within a couple weeks. He went from I don't have it (first test), to running more tests and saying not yet.

In the meantime, here are the changes.

Kappa Free 1.0 (0.33-1.94 mg/dl)
Lambda Free 4.04 (H) (0.57-2.63 mg/dl)
K/L Free Ratio 0.25 (L) (0.26-1.65)

Now what does this mean? I know that after the next test, depending on what he says, we are probably going to get a second opinion.
