Hello. A big thanks to everyone on this board as I've learned a great deal since my mother was first diagnosed a year ago with stage III multiple myeloma. She has lambda IgA with 2 monoclonal proteins (currently at 0.6 gm/dl and 0.2 gm/dl).
My question: Her most recent protein electrophoresis test report has a new line item - Monoclonal 3 Prt 0.2 gm/dl. Is this a 3rd M-spike? (I tried searching the internet about this and nothing came up and haven't asked my mother yet.)

As an update, I did find out that this is a third M-spike. I also discovered via this forum that it's not a prognostic factor and not uncommon. (Please correct me if I interpreted the comments wrong.) However, is a third M-spike an indication that treatment should resume?
Her last treatment ended in April, 2017 when she was alternating 5 mg and 10 mg Revlimid every other day along with dexamethasone on days 1, 8, 15 and 22.

My understanding is that it's not so much how many different M-spikes one may have, but rather the sum total of the M-spike values (i.e. the overall monoclonal protein burden) that would influence the decision to reinitiate treatment. However, I'm not a doctor and I've just picked up this understanding from reading various posts on the forum over the years. So, please defer to your mother's doctor on this point.

Thanks Multibilly! You provide great information on this site. This is what I thought from my research too. I'll tag along with my mother on her next appointment to hear what's happening. (She also has CMML which seems to be pushing her white cell count and monocytes pretty high; perhaps they're shifting gears and focusing on that cancer instead.)
