I had an ERCP [endoscopic retrograde cholangiopancreatography] for abdominal pain. It was normal but they sphincterized, dilated, and had a stent placed to prevent post procedure pancreatitis.
All was well for eight months when I got suddenly septic with Streptococcus Anginosis. I turned out to have a huge liver abscess. The bloods, drawn when they did not know what was wrong showed a CRP of 167. They followed with an SPEP which showed an M protein of .6 grams. IFE is IgG lambda; free light chains normal and ratio normal.
I had 6 weeks of IV antibiotics and saw Heme who did not want to do anything at that time and scheduled me for bloods two months later. I had an US of liver before the bloods and liver is healed.
Bloods showed same M spike but now 1 gram. My gamma globulins at diagnosis of M spike was only 0.9 total and now its 1.4 total so that doesn't worry me. I feel fine, am back to work full time, and exercising well. Immunoglobulin levels, liver tests, CRP and kidneys and calcium all normal.
What I am worried about, though, is the free light chains; the kappa is actually low and the lambda has fallen, too but still in normal range. This makes my ratio too low.
Should this worry me?
I don't think this ratio applies when your values are low and normal, respectively but I see stuff on the computer that says either a high or low ratio is bad.
