MGUS risk of progression and cytogenetics

Hey Guys

Wondering if there is a relationship between the chances of progression and genetic abnor­mal­ities (cytogenetics), in particular the +1q abnormality?

I have an abnormal free light chain (FLC) ratio (0.05) with an excess of lambda free light chains. Previous flow cytometry showed 94% of my plasma cells to be abnormal.

At the moment I have around 5% plasma cells in the aspirate and 8% in the trephine, which actually seems better than previous bone marrow biopsy (BMB), which was thought to be 7% in the aspirate and 10% in the trephine.

Is there any chance at all that I may not progress in view of the above? I was diagnosed with my plasma cell dyscrasia 2 years ago.

Hoping someone can help, I am aware that this is not an exact science, but would be so grateful to hear your thoughts on this.

Blessthischick,

There was a recent article on smoldering multiple myeloma and the impact of cytogenetics on the risk of progression.

SV Rajkumar et al, "Smoldering multiple myeloma", Blood, May 14, 2015 (full text of article) (related Beacon forum thread)

In particular, look at Table 3.

1q+ (aka 1q gain) was determined to be a high progression-risk factor for smoldering patients in this paper. But I don't recall seeing a similar study for MGUS patients. Also keep in mind that the assertions with respect to cytogenetics in this paper were based on just one study.
