MGUS or multiple myeloma?

by RWhiles33 on Tue Jan 09, 2018 3:10 pm

Hope it's okay to run these figures in regards to my MGUS / multiple myeloma.

IgG is 2585 (700-1600)
IgA 232 (61-437)
IgM 50 (15-143).

Red Blood Cells 4.26 (4.40-5.80)
Hemoglobin 13.8 (13.8-17.2)
Hematocrit 41 (41.0-50.0)
White Blood Cells 4.4 (3.8-10.8)
Kappa Light Chain Free 29.7 (3.3-19.4)
Kappa/Lambda Ratio 2.52 (0.26-1.65).

Under test interpretation it says:

"In serum, the Kappa/Lambda ratio of whole immunoglobulin molecules is 2:1, whereas the Kappa/Lambda ratio of free light chain is 1:1.5".

I don't understand this which ratio is the most relevant.

Glucose 94
BUN 25 (3-29)
BUN/Creatinine Ratio 23 (7-25)
Total Protein 8.1 (6.0-8.3)

24-Hour Urine test says:

"No monoclonal Proteins Identified."

Bone Marrow test states:

"Demonstrates involved kappa clonal plasma cells in the amount of 5%."

My current symptoms include somewhat low energy levels, numb feet (peripheral neuropathy), rather constant low level headaches, and shrinking muscle mass in shoulders (bone loss?). I am currently exercising 4-5 times per week using weights and elliptical machine while limiting exercise time to 30 minutes per workout. I am 84 years old.

I am still not clear if the above details would indicate what my "M-spike" is. I don't see any mention of "M-spike."

Are the above symptoms suggestive of multiple myeloma, or can they still be classified as MGUS?

I will be meeting with my hematologist in nine days. I'm trying to understand all of this.

Thank you.

Robert

Hi Robert,

Do you know if all of your results have come back? I don't see a serum protein electrophoresis (SPEP) test result, which I think is where an M-spike or monoclonal protein level would show up. When I had all my testing done, that was the one that took the longest to come back (about a week), so you might still be waiting for it. I think your doctor will want that number as part of his interpretation.

I'm new here, too, so I can't offer too much more. The bone marrow biopsy result of 5% clonal cells fits within MGUS criteria, but I think you need more data.
