MGUS IgM kappa + IgM MU heavy chain - need more tests?

16 years ago diagnosed small fiber neuropathy idiopathic with elevated IgM polyclonal immunoglobulins.

2 years ago serum immunofixation showed 1 IgM kappa protein and tests for binding neuronal antibodies to M protein were negative. MGUS.

6 months later I was told two distinct IgM kappa proteins seen on serum immunofixation and that I had markedly elevated binding neuronal antibodies to M protein and also autonomic neuropathy.

Repeat test I was told now show 1 IgM kappa protein and 1 MU heavy chain on serum immunofixation.

I am being followed by neurologist who said unless protein levels increase no need to see hematology. I was not told any level of the MU , and don't have records yet as recent test. Not sure how levels are determined for MU?. IgM kappa MGUS told stable only detected on serum immunofixation. 2 years ago had bone survey, abdominal ultrasound. Negative. Osteoporosis.

The tests being done since on follow up have been serum/urine quant. immunoglobulins with electrophoresis / immunofixation, neuronal antibodies, free lights. Told urine and free lights normal.

The question I have is about the changes on test findings over a short timeframe and whether other protocol tests are required for follow up in this kind of scenario when you show both IgM kappa and Mu heavy chain findings.

Hello from gray and cool Seattle,

I think it would be worth your time to see a hematologist for more information and possibly more evaluation of these findings. Even with "low" levels of the M-spike, some patients can have injury to kidneys, bones, etc.

I also felt I should see a Hematologist for more input . I just wondered what the changes on the test result can allude to?
