MGUS with high gamma globulin, kappa, and K/L ratio

Hi All,

I just received the following lab results:

Total Protein - 84
Gamma Globulin - 24.6
Free Kappa - 223.4
Kappa/Lambda Ratio - 25.98

I was diagnosed with MGUS some months ago. I am seeing my doctor next week and am wonder­ing how concerned I should be.

Hello Concerned,

I'm assuming the Total Protein and Gamma Globulin results were from a serum or urine protein electrophoresis test. Did the results of that test also report an "M-spike" (paraprotein, monoclonal protein, abnormal protein, abnormal protein band, etc)? If so, what was the level of the M-spike? Please include units and, if available, reference ranges with all lab results. Please also indicate whether the tests are blood or urine tests.

Did you have a serum immunofixation test done and, if so, what was the result?

Finally, have you had similar tests before, especially the free light chain test? If so, when was that test done, and what were the results?

The answers to the questions I just listed could help us get a better idea of the significance of the results you shared.
