Question please:
Kappa / lambda ratio = 8.80 in patient with 28-year history of chronic renal disease.
Kappa Lt. chain =129.85
Free Lambda lt. chain =14.70
M-spike = 0.3 / not observed.
Are the above reflective of MGUS or active multiple myeloma?
Thanks

Re: MGUS or active multiple myeloma?
Msrine4,
Welcome to the forum.
One cant' tell from just this data. You would also need to look for myeloma "CRAB" criteria (hypercalcemia, renal insufficiency, anemia and bone lesions) on a CBC & metabolic panel test as well as on your imaging results (MRI, PET/CT, full body xray, etc), etc.
Also, have you had a bone marrow biopsy?
Also, you doctor would need to determine if you've had any renal impact as a result of your monoclonal gammopathy or if all of your renal issues are due exclusively to your pre-existing CKD.
Lastly, are all these test results you mentioned so far from blood tests or from urine tests?
In any case, if you've had the tests performed you already listed, your doctor must be suspecting some sort of monoclonal gammopathy. Do you have a current diagnosis from your doctor and have you seen a hematologist that specializes in multiple myeloma?
A little more background on what led up to this would help folks on this forum better help you.
