I am finally getting round to an introduction, though I have already posted in this thread:
"Kappa / lambda ratio & myeloma defining events" (started Aug 11, 2015)
I was diagnosed with CLL (chronic lymphocytic leukemia) in 2014, had normal FISH, no issues, 20% marrow infiltration. Then in January 2018 they also found another clonal population of lymphocytes, with a different flow cytometry signature than the CLL, but a small (less than 5%) population in the marrow.
Over a year ago, after my second marrow biopsy, it was discovered there was a 50% marrow population of clonal plasma cells. Due to the way they looked under the microscope, it was assumed they were lymphoplasmacytic cells (similar to Waldenstrom's macroglobulinemia).
But in June 2018 a CBC found my eGFR dropped from 87 to under 50 and my creatinine increased by 50% or so to around 1.57 mg/dl. So, another marrow biopsy and flow cytometry testing on the plasma cells revealed t(14;16), gain 1q, del(13q), 50% marrow infiltration. This trumps the CLL and now the combined marrow impact is around 70% or higher.
I will be meeting later this month with a myeloma specialist to discuss treatment. My cytogenetics, particularly t(14;16) and gain 1q, are a high risk combination per the updated Mayo Clinic "mSmart" guidelines, and my doctor already has discussed the possibility of a tandem transplant in a discussion we had a month ago, but we'll see what her thoughts are when I meet with her this month.
