M Spike/paraprotein level

Just wondering if the M spike is the change in paraprotein level or is the paraprotein level.

It is the paraprotein level, not the change in the level. The terms M spike, paraprotein, M protein, and monoclonal protein are often used to describe the same thing -- the concentration of myeloma antibody in blood or urine. Certainly more complicated than it needs to be!
