Odd M-spike developments post stem cell transplant

My husbands myeloma is IgA lambda.

Post transplant he has had 3 IFE's.

At day 100, there was a 0.1 M spike of a DIFFERENT type (IgM band), but negative for the original protein.

At day 160, SPEP negative IFE was negative for original protein as well as the previous IgM band.

Then at the last blood test: SPEP negative and IFE negative for original IgA band, but showing 2 trace IgM bands too low to quantify.

Any thoughts on these weird bands?

This condition is called secondary monoclonal gammopathy of undetermined significance (MGUS). It is diagnosed when we find a new monoclonal (M) protein (band) during the course of multiple myeloma that has an isotype (heavy and/or light chain) which is different from the original type noted at the time of diagnosis (for example, development of new IgM MGUS in a patient with IgA multiple myeloma as you have noticed in your husband's case).

Secondary MGUS is more common among patients who have undergone stem cell transplantation, and has been found to be associated with a better survival. In nearly half of the patients with this entity it resolves without treatment in about six months.
