Light chains increasing while M-spike dropping

by Duchess of Pain on Thu Feb 16, 2017 6:33 pm

Hi,

My husband was diagnosed with multiple myeloma in December 2015. In late July 2016 he had a stem cell transplant. He is currently on 10 mg Revlimid daily for maintenance.

I have included below his test results since just before his transplant and up to January. The oncologist said she has no idea why the light chains are creeping up and the M-spike going down. Didn't offer anything to bring the chains down. My husband is concerned as to if she is up on the latest.

I know we measure things differently in Canada than the States, at least for the M-spike (i.e., Canada 17, States 1.7). It has been 14 months and I still feel I don't understand a lot of it.

DATE LIGHT CHAINS M-SPIKE
JUN 2016 34.00 17
AUG 2016 3.83 16
OCT 2016 8.26 15
NOV 2016 6.53 18
DEC 2016 7.68 13
JAN 2017 9.22 10

I looked in his charts and it says M spike is g/L measurements, whatever that is. Same with the IgG. The I gG was always up and the IgA and IgM were always low.

Hi Duchess,

Could you post both the kappa and lambda free light chain values, including the kappa/lambda ratio? It would also be great if you included the normal reference range for each of these numbers so we can be sure that we are looking at the appropriate test results.

Also, what were his latest IgG, IgA and IgM values (again, including the normal reference range for each)?
