Kappa & lambda light chain levels & colds and viruses

Hi Everyone,

It has been a few months since my first post where I talked about my Mom's first relapse. She relapsed for the first time in May 2016 after being in remission for about 2 years.

She was put on the Kyprolis, Revlimid, and dexamethasone (KRD) regimen, and she was doing very well. In 4 cycles her kappa light chain level went from 2200 to 396, which her oncologist was very happy to see. However, after cycle 5, her light chain levels jumped to 799, but all other levels remained normal.

Of course this concerns us. Does this mean she has relapsed again? Has Kyprolis failed?

We spoke with her oncologist and he wants her to do at least one more cycle of KRD and reassess at that point if she has in fact relapsed.

She was battling – and still is a bit – a pretty bad virus/cold for the past 2 weeks. We asked if this could have any impact on the light chain Levels, and her oncologist said its possible, but he is concerned about the significant jump.

We hope that this has to do with the virus/cold, but we are cautiously optimistic.

Is it possible that her light chains levels jumped quite a bit from a cold or a virus? Does it happen often? And will the levels go back to normal?

Hello Una,

Yes, I had a very similar experience early in my Revlimid, Velcade, and dexa­metha­sone (RVD) induction phase last year in March-May, 2015.

My kappa level was coming down rapidly and it had dropped to 24.8 mg/L from 1,070 after 3 cycles of treatment. I had a nasty viral flu and about 5 days of high temperature during the 4th cycle. My kappa free light chain level jumped by 100% to 45 (very similar to your mom's going from 396 to 799). My RVD treatment was stopped for a week during that cycle. However, my kappa dropped back to 24.4 mg/L after the next (5th) cycle.

My kappa / lambda ratio jumped up from 2.54 to 3.85 at that time and it, too, dropped back to 2.37 after another cycle.

In relative terms, my experience was similar, and I hope your mom's kappa follows the same pattern and returns to much lower levels soon.

My mother's kappa free light chain level jumped from 230 to 540 mg/l after a viral infection that hit her throat for 15 days.

Her lambda free light chain level jumped from 6 to 23.

Just hope the numbers settle back. She is on maintenance (cyclophosphamide) after 6 cycles of CyBorD (cyclophosphamide, bortezomib, and dexamethasone), to which she had responded well, with her kappa level dropping from 1150 to 230.
